Effective December 1, 2022, ECF e-filers in individual debtor cases must use the event "Statement of Social Security Numbers/SSN (Restricted Access)" to file Official Form 121, Statement About Your Social Security Numbers immediately after case opening. In compliance with the federal judiciary's privacy policy, the PDF image of the document containing the debtor's complete social security or other individual taxpayer identification number will not be available for public viewing. Failure to file Form 121 will result in the issuance of a deficiency notice by the Clerk's Office. Attorney filers are asked to please contact their bankruptcy petition preparation software provider to update the software to include the filing of this official form separate from the petition.
